Neuroimagen funcional en el pronóstico evolutivo de una cohorte de pacientes portadores de trastorno afectivo bipolar: estudio preliminar a partir de la técnica de neuroSPECT

Resumen

El Trastorno Afectivo Bipolar (TAB) es una enfermedad psiquiátrica, diagnosticada clínicamente según los criterios de DSM IV y/o CIE 10. Estudios recientes postulan que en su fisiopatología existe un componente orgánico, demostrado mediante el SPECT cerebral. La diversidad del patrón imagenológico obtenido con dicha técnica en estos pacientes también podría ser utilizada para obtener un pronóstico evolutivo de la enfermedad. Un total de 63 pacientes reclutados y tratados en el contexto de la práctica privada de uno de los investigadores cumplían con los criterios clínicos de TAB y la corroboración neuroimagenológica de neuroSPECT. Los resultados obtenidos de los estudios SPECT fueron analizados observando la perfusión en las diferentes áreas de Brodmann. Las imágenes de SPECT cerebral se correlacionaron exitosamente con la severidad clínica del TAB. Se concluye que la técnica de SPECT cerebral puede utilizarse para confirmar el diagnóstico y establecer una correlación objetiva de la evolución de la enfermedad, que permita adecuar el tratamiento a su severidad.

Abstract

Bipolar Affective Disorder (BAD) is a psychiatric illness, diagnosed clinically according to DSM IV and/or CIE 10 criteria. Recent studies on its physiopathology postulate that there is an organic component that has been demonstrated by SPECT. The diversity pattern in the images obtained with this technique could also be used to obtain a prediction of the evolution of this disease. A total of 63 patients enrolled and treated in the context of private practice of one of the researchers, met the clinical criteria of BAD and neuroimaging corroboration with neuroSPECT. SPECT results were analyzed noting the perfusion in the different Brodmann areas. SPECT images were correlated successfully with the clinical severity of the BAD. SPECT technique can be used for confirming the diagnosis and for establishing an objective correlation of the evolution of the disease that can adapt treatment according to its severity.
